The federal government is funding a sea change in health care by investing in interventions targeting social determinants of health, which are significant contributors to illness and health inequity. This funding power has encouraged states, professional and accreditation organizations, health care entities, and providers to focus heavily on social determinants. We examine how this shift in focus affects clinical practice in the fields of oncology and emergency medicine, and highlight potential areas of reform.

People prefer linguistic stimuli with an inward (e.g. BODIKA) over those with an outward articulation dynamic (e.g. KODIBA), a phenomenon known as the articulatory in-out effect. Despite its robustness across languages and contexts, the phenomenon is still poorly understood. To learn more about the effect’s boundary conditions, mental representation, and origin, we crossed the in-out effect with evaluative conditioning research. In five experiments (N = 713, three experiments pre-registered), we systematically paired words containing inward versus outward dynamics with pictures of (...) negative versus positive valence. Although this evaluative conditioning procedure reversed the preference for inward over outward words, this was the case only for words with the same consonant sequences as the conditioned words. For words with inward/outward dynamics but different consonant sequences than the conditioned ones, a regular in-out effect emerged. Also, no preference reversal at all emerged for the conditioned consonant sequences when the contingency between single consonants at specific positions and positive/negative valence was zero. Implications of these findings for the in-out effect and evaluative conditioning are discussed. (shrink)

This article contains the principal ideas that I presented in four different sessions at the IRAS 2022 conference, on the theme “‘We' and ‘They’: Cross-Cultural Conversation on Identity.” Focusing on the central topic, the article begins with (i) the contents of my opening lecture; followed by (ii) a broad outline of the concerns discussed in my book, Cross-Cultural Conversation: A New Way of Learning, intertwined with glimpses of the intellectual journey that led me to CCC, delivered in the Book-discussion session; (...) There is evidence that many peptide growth factors and hormones act in the intracellular space after either internalization or retention in their cells of synthesis. These factors, commonly called intracrines, are structurally diverse while sharing some common functional features. Reports of intracellular peptide hormone binding and action are reviewed here. Also, this laboratory has made proposals regarding the origin and actions of intracrines and these areas are further explored. Intracrine interactions and the relationship of intracrines to transcription factors are discussed. (...) Background In May 2020, England moved to an opt-out organ donation system, meaning adults are presumed to be an organ donor unless within an excluded group or have opted-out. This change aims to improve organ donation rates following brain or circulatory death. Healthcare staff in the UK are supportive of organ donation, however, both healthcare staff and the public have raised concerns and ethical issues regarding the change. The #options survey was completed by NHS organisations with the aim of understanding (...) awareness and support of the change. This paper analyses the free-text responses from the survey. Methods The #options survey was registered as a National Institute of Health Research (NIHR) portfolio trial [IRAS 275992] 14 February 2020, and was completed between July and December 2020 across NHS organisations in the North-East and North Cumbria, and North Thames. The survey contained 16 questions of which three were free-text, covering reasons against, additional information required and family discussions. The responses to these questions were thematically analysed. Results The #options survey received 5789 responses from NHS staff with 1404 individuals leaving 1657 free-text responses for analysis. The family discussion question elicited the largest number of responses (66%), followed by those against the legislation (19%), and those requiring more information (15%). Analysis revealed six main themes with 22 sub-themes. Conclusions The overall #options survey indicated NHS staff are supportive of the legislative change. Analysis of the free-text responses indicates that the views of the NHS staff who are against the change reflect the reasons, misconceptions, and misunderstandings of the public. Additional concerns included the rationale for the change, informed decision making, easy access to information and information regarding organ donation processes. Educational materials and interventions need to be developed for NHS staff to address the concepts of autonomy and consent, organ donation processes, and promote family conversations. Wider public awareness campaigns should continue to promote the positives and refute the negatives thus reducing misconceptions and misunderstandings. This paper has three aims. First, I defend, in its most radical form, Hume's scepticism about practical reason, as it applies to purely self-regarding matters. It's not always irrational to discount the future, to be inconstant in one's preferences, to have incompatible desires, to not pursue the means to one's ends, or to fail to maximize one's own good. Second, I explain how our response to the “irrational” agent should be understood as an expression of frustrated sympathy, in Adam Smith's (...) sense of sympathy, rather than a genuine judgement about Reason. We judge these people because we cannot imaginatively identify with their desires and attitudes, and this is frustrating. Third, compared to the standard cognitive view, my account better explains the nature of our criticism of the “irrational,” and, by portraying “irrationality” as a cause of upset to other people, provides a better normative basis for being “rational.”. Recent democratic theorists have drawn on the work of the late Pierre Bourdieu to make the case that patterned inequalities in the social capacity to engage in deliberation can undermine deliberative theory’s democratic promise. They have proposed a range of deliberative democratic responses to the problem of cultural inequality, from enabling the marginalised to adopt the communicative dispositions of the dominant, to broadening the standards that define legitimate deliberation, to strengthening deliberative counter‐publics. The author interprets Bourdieu’s theory of the linguistic (...) habitus to prompt an even more radical critique of deliberative democracy than these theorists acknowledge, one to which the proposed solutions fail adequately to respond. Her argument suggests that empirical work on deliberative democracy should expand to address specifically the problems of cultural inequality that Bourdieu’s work highlights. (shrink)

An important question about eye-movement behavior is when the decision is made to terminate a fixation and program the following saccade. Different approaches have found converging evidence in favor of a mixed-control account, in which there is some overlap between processing information at fixation and planning the following saccade. We examined one interesting instance of mixed control in visual search: lag-2 revisits, during which observers fixate a stimulus, move to a different stimulus, and then revisit the first stimulus on the (...) next fixation. Results show that the probability of lag-2 revisits occurring increased with the number of target-similar stimuli, and revisits were preceded by a brief fixation on the intervening distractor stimulus. We developed the Efficient Visual Sampling computational model to simulate our findings and to provide insight into mixed control of fixations and the perceptual, cognitive, and motor processes that produce lag-2 revisits. What they do matters enormously, significantly determining the kind of society we have now and will have in the future.Universities, as former Harvard president Derek Bok and others have emphasized, have become the central societal institutions in the world (Bok, Universities and the Future 3). Research universities, in my judgment, are the most central. They develop new ideas and technologies, incubate businesses, serve as cultural and artistic centers, and are engines of local, national, and global economies. As anchor institutions, they often engage in partnerships with government, the private sector, and community-based organizations to revitalize local neighborhoods and schools (“Anchor Institutions Task Force”). It is the university’s role as an educational institution, however, that is most important.The schooling system increasingly functions as the core subsystem—the strategic subsystem—of modern information societies. Schooling, more than any other subsystem, as John Dewey claimed, influences the functioning of the societal system as a whole. If human beings hope to maintain and develop a particular type of society, they must develop and maintain the particular type of education system conducive to it. In an 1899 speech at the University of California, he perceptively observed that “[t]he school system, the character of which, in spite of itself, the university determines and in a large measure controls.... This book provides a distinctive and rich conception of methodology within international studies. From a rereading of the works of leading Western thinkers about international studies, Hayward Alker rediscovers a 'neo-Classical' conception of international relations which is both humanistic and scientific. He draws on the work of classical authors such as Aristotle and Thucydides; modern writers like Machiavelli, Vico, Marx, Weber, Deutsch and Bull; and post-modern writers like Havel, Connolly and Toulmin. The central challenge addressed is how to integrate (...) 'positivist' or 'falsificationist' research styles within humanistic or interpretive ones. The author argues that appropriate, philosophically informed reformulations of conventional statistical and game-theoretic analyses are possible, and describes a number of humanistic methodologies for international relations, including argumentation analysis, narrative modeling, computational models of political understanding and reconstructive analysis.
